About 25% of adults have Genital Herpes Hsv-2. In 95% of cases oral herpes is caused by hsv-1. 5% of oral infections are caused by hsv-2 though.
BECAUSE 80% of us already have the virus hsv-1 on the mouth, genital hsv-1 is not as common as oral hsv-1 – once you have hsv-1 on the mouth and have had it a while, you CANNOT catch hsv-1 on the genitals (although you can still catch genital hsv-2).
So, if two people both have cold sores caused by hsv-1, they are extremely unlikely to spread the virus to each others genitals through oral sex.
However, if within a couple one person gets cold sores and the other doesn’t, the person who doesn’t have oral herpes can catch genital herpes if their partner gives them oral sex – even in between cold sores. In fact, a person that has oral herpes is infectious about 10% of the time when they do not have a cold sore.
Neither I nor my sister caught cold sores as children, as is typical. So neither of us had oral herpes hsv-1. Consequently, we both caught genital herpes hsv-1 from partners that did have oral herpes giving us oral sex – in both cases when they were ‘in between’ cold sores. Of the four women I have known, including myself, that have genital herpes, all have hsv-1 rather than hsv-2 and all caught it through oral sex.
Most people seem to think that spreading herpes from the mouth to the genitals is relatively uncommon. It isn’t! In fact, hsv-1, which is normally spread to the genitals from an oral infection, now accounts for 50-70% of genital herpes infections as well as 95% of oral infections.
This is partly because not as many kids catch oral herpes as they used to – among current teenagers only about 50% have it, which means the other 50% can catch hsv-1 genitally if their partners that get cold sores give them oral sex.
How can you prevent it? Well unless you either abstain from giving oral sex or your partner has the same oral herpes virus as you, you can’t.
Being careful to avoid giving oral sex when you have ANY symptom reduces the risk – you should avoid contact from the first tingle to at least 5 days after sores have completely healed – but even so you can still pass it. My boyfriend and I were well aware that his cold sores could give me genital herpes, but we thought if we were scrupulous about avoiding every symptom we would be ok. He still gave it to me, when he hadn’t had symptoms for months.
The choice is either to abstain from oral sex, use a dental dam or condom, or avoid the riskiest times and accept that a small risk is always there, as I did, but that you may well catch it/pass it.
